Sacred Life Hospice Services

24 hours a day availability: A fully staffed On-Call Team providing supportive care 24 hours a day. A hospice Medical Director/ Attending Physician/Nurse Practitioner Services: These professionals are responsible for the palliation and management of the terminal illness and conditions related to the terminal illness.

A Registered Nurse Case Manager is assigned to each hospice patient to foster a trusting relationship and for continuity of care. The RN Case Manager is responsible for providing initial and ongoing assessments of the patient and communicating these needs to the Medical Director, Attending Physician, or Nurse Practitioner. Licensed Vocational Nurses work collaboratively with the RN Case Manager to provide nursing services according to the patient’s plan of treatment or plan of care.

Nurses provide compassionate, empathetic care and education to the patient and their family members. The nurses are specialized in providing palliative care for pain control and symptom management.

A qualified Social Worker is provided to each patient. Social workers provide a comprehensive psychosocial assessment and interventions based on the patients and families needs. They can provide assistance with obtaining advanced directives, outside care giver support, mortuary plans, and other matters of financial concern.

Certified Home Health Aides assist with performing personal hygiene and grooming tasks for patients. Homemaker services are available for providing light housekeeping and maintaining a clean, safe, and healthy environment.

Spiritual Counselors provide assessment of the patient’s and family’s spiritual needs and can facilitate visits by local clergy in order to best meet the patient’s spiritual needs. Bereavement services are available for family members and other individuals for up to thirteen months following the death of a loved one.

Specially trained Volunteers are available to provide companionship and caregiver relief to the patient and their family members.

Medications, medical supplies, and durable medical equipment related to the palliation and management of the terminal illness and related conditions are provided.
